Information you may choose to share

The consultation form asks for your name, contact number, optional email address, area of interest and optional preferred callback time. A professional referral also asks for the practice or clinic name and the patient’s name.

Please share only what is needed for the enquiry. Do not enter medical reports, government identification numbers, payment details or sensitive clinical notes in this form.

How the booking form works

The details you type remain in your browser while you complete the form. After you select “Review enquiry,” the website prepares a message for you. Nothing is sent at that point.

If you choose email or WhatsApp, the selected app or service opens with the message ready. You must review it and press Send there. Information sent this way is also handled under the privacy terms of your email provider or WhatsApp.

Why LensWise uses the information

LensWise uses enquiry details to respond, discuss your needs, arrange a suitable consultation, manage a professional referral and keep a reasonable record of the conversation. Submitting an enquiry does not confirm an appointment or establish a clinical relationship.

Sharing, storage and retention

LensWise does not sell booking-form information. Information may be processed by the communication service you choose and by service providers needed to operate and secure those communications.

Enquiry records are kept only for as long as reasonably needed to respond, follow up, manage appointments and referrals, or meet legitimate administrative and legal requirements. Reasonable steps are taken to protect information, but no internet or messaging method can be guaranteed completely secure.

Referrals and children

Referring professionals should confirm that they have the patient’s permission before sharing identifying details. A parent or legal guardian should submit or approve enquiries concerning a child.

Website services and external links

The website may create basic technical records needed for delivery and security. The About page embeds Google Maps, and links may open Google Maps, email, WhatsApp or other external services. Those providers may receive technical information and apply their own privacy policies. The website does not currently use booking details for advertising.

Your choices
